HUB21 DATA SUBJECT APPLICATION FORM

GENERAL INTRODUCTION

It is envisaged for data subjects (from here on referred to as “Applicant”) defined as the

relevant person by the Personal Data Protection Law numbered 6698 (“PDP Law”) to make

some requests on processing of their personal data in compliance with the PDP Law Article 11.

According to the PDP Law Article 13 paragraph one; the applications regarding these rights

shall be forwarded to the controller HUB21 Learning UK Limited (“Company”) in written or

in other means determined by the Board of Personal Data Protection (“Board”).

In the light of the aforementioned, the application can be submitted by printing out this form

and through;

• the Applicant’s own application for his/her name

• Notary

• Signing with the secure electronic signature envisaged by the Electronic Signature Law

numbered 5070 and sending it with an e-mail to the Company’s e-mail address

info@thehub21.com.

• Applicant sending an e-mail from the registered e-mail address which is previously

declared to our Company.

Below, the channels of written application are defined by explaining how to forward the written

applications to us. We would kindly request for you to follow the detailed steps that are stated below.

The other information of your video, photo and audio recordings are collected during your use of

the services.Why do we collect your personal information?

It is very important for us to provide you and your child a safe and secure environment. So, we do

not want anyone who could use the Websites in harm way to be in the same online environment

with you. For this and many more reasons, we want to identify the people who use our services.

In order to carry out this identification we require everyone to provide their previously mentioned

We also need for you to create a username on your childs behalf to be identifiable and have your

personal account on the Website. Your password is for your own personal access to the Website
